I understand that the results of any coaching depends on my ability to take action and follow the coaching plan to the best of my ability. I take full responsibility for the results achieved, knowing that the coach can only facilitate.

I agree to communicate clearly with the coach on all issues that I need assistance with. Issues not shared with the coach may not be resolved. This includes issues with the coach. The coach agrees to use best efforts to assist resolving issues (with confidentiality).

I agree that the coach may assign tasks to be done in between sessions and I will give my best effort to fulfill those tasks.

This agreement starts with the first session. It can be cancelled with 30 days of notice, given in writing.
